Offer guidance, direction, education and monitoring | |

Follow up with requirements outlined in AFP site review (i.e., SD plan and implementation and PCP external facilitation) | |

Keep up to date with DCH contract requirements and performance objectives, Medicaid Chapter III changes and monitor compliance | |

Update policies/procedures and trainings as needed to ensure best practice | |
|
Act as information broker within the Affiliation and other pertinent agencies | |
|
Chair the Self Determination work group | |
|
Attend management/team meetings within the Affiliation as necessary/requested, such as Affiliation Core Group meetings, Consumer & Stakeholder Advisory Council meetings, etc. | |
|
Standardize information and education as much as possible within the Affiliation while allowing for individuality between the different agencies | |
|
Be able to visit Affiliate agencies as often as necessary/requested and keep a flexible schedule to offer guidance as needed/requested | |
|
Allow for direction and integrate pertinent recommendations from Affiliate leadership into the role on an ongoing basis | |
|
To promote PCP, SD and consumerism across the Affiliation by supporting the efforts of each Affiliate member Board and providing consultation/guidance for designated staff within each Affiliate |
